Non-Contributory:

Basic Life Insurance

Adelphi will provide employees with basic life insurance in the amount of 1x annual salary ($50k minimum and $500k maximum). The IRS considers the value of group term life insurance in excess of $50,000 as income to an employee. The imputed cost of coverage in excess of $50,000 must be included in income.

For APPWLU, Public Safety and Local 1102 employees, your basic life insurance benefit will remain in the amount of $50,000. Please see your respective collective bargaining agreements.

Contributory:

Optional Life Insurance

You can purchase up to 3 times your basic annual earning up to a maximum of $500,000*.

Accidental Death and Dismemberment (AD&D)

This insurance provides your beneficiary with added protection in case of your accidental death and/or a benefit payable to you in case of a dismemberment.* Coverage will be available if you elect contributory life insurance. The amount of coverage is the same as the amount of coverage for contributory life insurance. Various lesser amounts are paid for dismemberment.

Dependent Life Insurance

This insurance is available to you as follows:

• You can purchase $5,000 of life insurance for a spouse.
• You can purchase $1,000 of life insurance for eligible dependent children.
• Coverage will be available if you elect contributory life insurance.

To increase your Life Insurance, please complete the EOI Application at www.sunlife-usa.net/eoi/. The Group Number needed for the application is 903900.

*Any amount over $300,000 is subject to Evidence of Insurability. If not elected within 31 days after date of hire, employee must provide EOI.
